A must from primary care/Hospitalist/nephrology
Never trust what someone tells you at face-value in medicine. Always do your own research. And when it comes to knowing about the kidneys, this is the book you have to own. I consider this book the golden standard for understanding and treating hypertension, AKI, CKD, DM with CKD, and even hyperlipidemia with CKD.If you ever wondered how to avoid AKI with IV contrast, the clear answers are in the text; do not trust someone when they say, ‘what’s the serum creatinine?’, when they determine if someone should have IV contrast; the text clearly states that serum creatinine is just one of many risk factors AKI after IV contrast, such as age, and multiple comorbidities; the real determinant is eGFR, and if yo undo not believe me read the book.As for what BP you should treat to, JNC 8, or AHA guidelines, the text does a not sway one way. The text explains each recommendation and why each came to their own conclusion. So never trust someone when they say, ‘use the AHA guidelines, b/c it’s newer’. When I hear that, I know the person speaking never read the studies. They all used the same information, except JNC8 only used the best studies; AHA stated there is not enough data on great studies, so they combined data on multiple studies that were not perfect. This is why medicine is an art, not just a science; every person is different, and you have to understand the guidelines before practicing them.
