Q: Will this work for Madden 21?

I wouldn't recommend it for any sports game that requires more than one control stick

Q: Does this actually work out of the box on ps4/xbox one, or does a controller need to be plugged in?

A controller needs to be plugged in it comes with a charging cable

Q: Does this work with PS5?

Yes, but there is a catch and its very annoying. Requires the use of a Dual Sense controller to do it. Use a certified PlayStation arcade stick from Madcatz and Qomba, it'll cut you off from other platforms, but it'll insure that when in PS4 mode that the PS5 can pick it up.

Q: compatible with windows 10? only direct and x-input?

Yes all it needs to do is setup as soon as you plug it in
