How is it mounted? There are literally NO instructions.

Seems to be the often coming from China. It does seem to have a variety pack of mounting pieces. Whether you have drywall, concrete or just need to use adhesive mounting. This comes with this item and you decided what works best with you. I used the drywall mounts for my plaster walls. It’s plastic hangers with nails you bury into plaster or drywall. Seems to work well.

Does it come with lights?

Yes. Ours came with a small set of lights.

Could this be used as a headboard for a bed?

No, not nearly sturdy enough on its own.
