How to use 1st time. Is IT requires to install fuse provided with it. I tried to use it 1st time with my Skoda rapid but unable to do so.

Fuse is already installed, the one you get additional is for replacement if original is blown. First you check that your power port is working by using any mobile charger or similar device. If port is working then please check fuse by opening of inflator power plug. If that is also ok, then you should raise a request for replacement, the unit may be damaged in transit. We are sorry for that. Product comes with one year warranty, so you have your peace of mind if product is gone bad.

We should operate it while engine is on or off

It depends on battery capacity of your vehicle, normally all mid and above range vehicles can run it with engine off, but if its not running you can run it with engine on for sure

Can I use the inflator with the engine running?

Yes. It is better to use with engine running so that the battery dosent get drained
