This game is handheld compatible, and compatible with the Switch Lite.

Q: What button is used to make the car go my son has little hands and has a tough time hitting the top buttons while playing

The L button is drift and the R button is accelerate. Those are both on top of the controller. This game is trickier as it does require good drifting to turn corners successfully. This game is great, but I think Crusin' Blast would be better for younger audiences (you can accelerate with the A button in that game.)

Q: on a normal switch can you play multiplayer with the joycons and if you can, how many joycons do you have to use per player?

It's only 2 players max, you can do split screen in the multiplayer settings.. only need one joy con per player. My kids have the steering wheels they can pop them in and pretend to be driving. If you play online, it's a total of 12 cars including yourself. 2nd player can't join online race or score achievements. It's fun but it is not like Mario Kart. This is not a large party game unless you all have your own devices and separate games and play online together

Q: How do you avoid having to purchase content? It seems like everything my kid wants to do requires me to purchase something...

You don't have to buy anything there. Just play what the game has to offer you. It just won't have as much content as it would if you bought the dlc (unfortunately).
