I bought this to help with breathing and the expanding of my lung after losing a third of it to cancer. So for me it was for inhaling, not exhaling.

Keep upright and suck on the tube trying to keep one or more balls up the tubes as long as possible. repeat to help your breathing. Turn the thing upside down on blow into the tube again trying to keep the balls up the tubes as long as possible. The whole idea is to improve your lung capasity.
