Q: Does this come with a cover?

There’s no “cover”, but there is a silicone sleeve for the tip.

Q: what size hole does it make?

I measured the widest part of the blade at 1.78 mm, but the hole it makes will depend of what material you are punching. Paper, leather, cloth (etc.) all close on the opening differently so the hole will always be less than 1.78 mm. I use mine to "scratch" (or score) leather when I'm transferring a pattern I'm working on. I don't believe you can find a tool that feels better in the hand than this awl for that purpose. If you want to make a hole in leather, I'd like to make some recommendation based on recent purchases. Both are sharp and accurate and leave a hole the size of the tool you are using.
