Q: Is it a fixed ND filter or adjustable? If fixed what is its density?

ND is not adjustable, I would check Hoya's site for the details.

Q: Does anyone know the quality type these filters are? such as hd3, evo, nxt, or alpha? in reference to ken rockwell's hoya filter types article?

The UV filter is labeled as HMC and the ND8 and CP-L have no designation on the filter exact slim. These are low end filters. I had no issue with the UV filter and the ND8 is good but the CP-L filter leaves purple color casts. I have since purchased the HOYA HD3 CP-L filter and it is outstanding but expensive.
