Q: How do I know when to replace this filter?

You don’t know. Only way to check us by removing the bottom cover (for TrueAir Odor Removal model) and check for dirt, dust, etc. This is my third purchase of this brand. First purchase lasted 2 months, 2nd and 3rd replaced after 5 weeks. The HB filters, while more expensive, last longer which justifies for me the higher cost.

Q: does this come with the indicator element?

no I paid extra and still did not receive them just the filters

Q: is this charcoal or something else?

it appears to have a filter side, a center with odor absorbing pellets and a black (activated charcoal) side. That is what it appears to me. Hope this helps. There is one NOTE to be made here, it will not be able to be washed. I tried it. The first time it did wash off the dust and grim on the blue filter side and a lot of nicotine/pollen washed out of it. BUT the second time I tried to wash it off, the edge GLUE disintegrated and the pellets came out. So I am assuming it should be used one time and replaced . Hope this helps
