Is the filter replacement light based only on passage of time or does it sense and use the actual condition of the filter to indicate when to replace?

The Filter Light Indicator is based on time and fan speed. When the unit is running on Auto, the fan speed will self-adjust as the Particle Sensor gages changes in the air. The filter life for the 75i is typically 12-15 months.

We have had the Purifier for 2 years and the filter replacement indicator still reads green? ...the purifier runs 24-7.

The duration of the filters for the BreatheSmart 75i units is from 12 to 15 months with constant (24/7) usage. The duration can depend on the conditions or the state of the air in your room. Every time when you change the filter you need to reset the filter indicator by pressing the filter button for 5 seconds. Hopefully, you didn't reset the filter indicator by accident during the mentioned period.

What should I do if I didn’t reset the filter light (it’s been 4 mos since new filter was installed)?

You may reset the filter light, but just notate that the filter has been used for 4 months. The next time you change the filter, you may reset the filter light and it will reset with the next new filter.
