Q: Is this just a bag or does it have some kind of treatment that kills them?

it's a big plastic bag, and also a pouch of chemical. It's difficult to fit the mattress in it. I ended up just using a hand-held steamer and that worked great.

Does not work well, had to trash everything to get rid of the pest

Q: How long do you keep the mattress in the bag?

It only needs to be in the bag for up to a week to kill eggs. I would do that just to he safe because you can't always see them. Then buy a bed bug cover for your mattress , box spring and pillows for future protection
