Q: Can I change the input/output setup so water goes in, turns 90 degrees, and then goes out?

Yes. The stock openings are set 180 degrees as shown. There are cutouts placed on the other two vertical surfaces, as well as on the bottom surface.

Q: I need an additional adapter for a three hole installation. Is there one available and what is part number?

I don’t know the part numbers but there are parts for that

Q: will this withstand car traffice in my driveway?

It should / it is a thick hard vinyl/ rubber - Mine is in a patio so it is only getting people walking on it

Q: How do you keep grate from coming off during hard rains?

Mine has 2 brass screws that hold the grate/top in place. I have never had a problem but mine has a 4" perforated drain pipe (40' long) attached.
